What you need to complete the application:

• Certificate of Completion for human subjects research training (in pdf format)
• Informed Consent Letter (as adapted for your project)
• Questionnaire, survey, focus group and/or interview questions
• Permission letters
• Dissertation Proposal for reference
• Any supporting documents you may be submitting with your application (e.g., confidentiality agreement)

You cannot save your application to finish at a later time. Please plan to complete it in one sitting. It is recommended that you prepare your answers in a separate word or text document prior to filling out the final application below. You may do so by downloading a copy of the application here or by working through this practice application. This will allow you time to formulate and proofread your responses before submitting the actual application.
